Synopsis – This scenario is designed to test a paramedic-level provider and their team in providing aggressive management to a COPD patient experiencing an exacerbation. Be aware that the treatment algorithm is based on University Hospital protocols (2015)
Directions - The various patient states are outlined below and should be advanced by the scenario operator in accordance to their descriptions.
Divulge information only after the provider has asked for it and once there has been sufficient time in real life to obtain that information.
If provider at any point chooses to perform Drug Assisted Intubation, proceed to State V – Drug Assisted Intubation.
The following actions will result in immediate transition to State VII – Death:
o Failure to initiate high flow oxygen, nebulizer, or CPAP within 5 minutes
o Failure to initiate transport within 20 minutes
o Discontinuing any appropriate therapy without suitable replacement
o Performing any life-saving intervention inappropriately
Objectives and Measures - Learning Objectives
o Determines scene safety
o Performs primary and secondary assessment
o Performs ongoing assessment
o Obtains patient history
o Recognizes signs/symptoms of COPD
o Recognizes signs/symptoms of Tension Pneumothorax
o Initiates oxygen therapy
o Initiates IV/IO therapy
o Transports patient within 20 minutes
o Perform treatment and drug therapy per University of Missouri EMS protocol
o Demonstrates appropriate communication and leadership to EMS team
Objectives and Measures - Performance Measures
State I – Initial Presentation
o Surveys scene for safety
o Identifies patient is experiencing respiratory distress
o Initiates oxygen within 5 minutes
State II – Non-rebreather/Nebulizer
o Administers oxygen therapy correctly
o Recognizes that therapy is insufficient
o Changes therapy within 5 minutes
State III – CPAP
o Recognizes need for in-line nebulizer treatment within 5 minutes
o Continues providing care per protocol
o Provider does not discontinue CPAP
State IV – Assisted Ventilations
o Recognizes that assisting ventilations is inappropriate
o Changes treatment therapy within 5 minutes
State V – Drug Assisted Intubation
o Manage patient’s airway appropriately for 5 minutes
o Appropriately administer drug therapy for induction if intubation is attempted
State VI – Tension Pneumothorax
o Performs needle decompression within 5 minutes
State VII – Death
o Performs needle decompression within 5 minutes
o Performs appropriate care for 5 minutes
State VIII – ROSC
o Performs appropriate care for 5 minutes
